Essential Duties and Responsibilities:
· Establishes collaborative, respectful, trusting and individualized relationships with patients.
· Provides case manager services to grant participants.
· Works closely with the syringe exchange referral source (COIP) and other harm reduction organizations to provide support for individuals identified at high risk for HIB who also have opioid use disorders (OUD).
· Conducts HIV screenings, provides HIV pre/post-rest counseling, completes MAT intake interviews and assessments and connects patients to the HIV service provider partners (UIH-Mile Square) for HIV treatment and PrEP services.
· Follows up with patients to ensure they remain engaged in SUD treatment and recovery support services as well as HI treatment and prevention services.
Schedule: Monday - Friday 6:00 AM - 2:30 PM; based on agency needs

Work Locations: 310 W. Chicago Ave., Chicago ( with occasional travel to 2630 S. Wabash Ave., Chicago, IL)
Pay Rate: Up to $40,000, depending on experience
