As an Assessment Unit Counselor (QMHP), you’ll play a vital role in helping individuals and families navigate behavioral health challenges and build healthier futures. You’ll work in a structured, weekday schedule while making meaningful clinical decisions, developing individualized care plans, and supporting clients through assessment, coordination, and intervention services.
In this role, you’ll have the opportunity to work independently while being fully supported by a collaborative care team.
Connect with newly referred clients and families within two business days, helping them feel supported from day one
Complete intake assessments and guide clients through program expectations and required documentation
Create person‑centered and family‑centered care plans tailored to each client’s unique needs
Provide therapeutic interventions, community support, care coordination, and crisis intervention
Monitor treatment progress and participate in service plan reviews to ensure effective, evolving care
Coordinate with internal teams and community partners to connect clients to meaningful resources
Maintain timely, accurate clinical documentation using the electronic health record (EHR) system

Strong clinical interviewing, assessment, and counseling skills
The ability to analyze client needs and adjust interventions based on progress
Knowledge of community resources and service systems
Comfort using EHR systems and virtual communication tools

Competitive Salary: $54,000–$72,000 (based on licensure & experience)
