About the Role
The Care Coordinator provides mentorship, coaching, and coordinated support services for participants navigating social, behavioral, and economic challenges including but not limited to, substance use, mental health concerns, housing instability, unemployment, and legal involvement. This role emphasizes community-based support and life-skills development, empowering participants to achieve stability, independence, and personal growth. Responsibilities include assessing participant needs, developing individualized support plans, connecting participants with social service and community resources, maintaining accurate documentation of participant activity and traveling regularly for participant meetings and other harm reduction activities.
Responsibilities
- Conduct assessments to identify participant strengths, challenges, and aspirations and develop individualized support plans that integrate recovery goals with life-skills development.
- Provide mentorship and assistance to participants in regards to their support plans to stay on target to achieve goals.
- Coordinate access for participants to healthcare, counseling, and other social services. Assist participants with scheduling, application completion and other tasks necessary to access services.
- Advocate for participants to overcome barriers and access needed resources.
- Connect participants with support groups, peer activities, and community events.
- Represent the organization at community events and build partnerships with local employers and service providers.
- Offer emotional support, crisis intervention, and guidance during challenging periods.
- Maintain accurate participant records, documenting progress notes, services provided, and outcomes to support program evaluation.
- Ensure compliance with confidentiality and ethical standards in all documentation.
- Travel regularly to participant meeting locations, including residences, court houses, administrative buildings, and community sites.
- Provide transportation support when necessary to ensure participants can access services and appointments.
Qualifications
- Exceptional organizational, interpersonal, and communication skills.
- Strong ability to build trust and mentor participants from diverse backgrounds.
- Compassionate, calming presence, especially for individuals in crisis; comfortable working with people experiencing homelessness, disabilities, or other challenges.
- Knowledge of harm reduction principles, recovery pathways, and community resources.
- Experience in nonprofit, public health, or community service settings preferred.
- Reliable, punctual, and adaptable in a fast-paced, mission-driven environment.
- Professional and respectful, with the ability to handle sensitive information confidentially.
- Passion for harm reduction, advocacy, and stigma-free support.
- Proficiency in EHR systems, documentation, and outcome reporting for grant compliance.
- Excellent attention to detail and ability to manage multiple priorities.
- Familiarity with healthcare systems, social services, and rural community challenges.
- Ability to work independently and collaboratively in a fast-paced environment.
- Valid driver’s license and willingness to travel regularly.

Compensation & Benefits
- Compensation for this role is $18-20/hour
