GENERAL FUNCTIONS: The Assistant Director of Transitional Programming is responsible for providing program oversight and for supervising a team of direct service staff who will link and provide services to clients upon discharge from psychiatric hospitalization. Recovery-oriented services that will be provided include, but not limited to assessment, advocacy, case management, community support, transitional housing and individual therapy as needed. AD will develop processes, procedures, and promotional material to help meet program deliverables identified by the state. AD is responsible for all reporting requirements and adhering to all state regulations and requirements per grant programs supervised. AD is further responsible for maintaining compliance with DHS, CARF, and other government regulations regarding programs associated with MRO billing.
ESSENTIAL FUNCTIONS:
· Responsible for participating in decision making on an administrative level in conjunction with the Director of Mental Health.
· Responsible for assisting with the development of policies and procedures, program planning, implementation and evaluation.
· Responsible for maintaining Key Performance Indicators/deliverables as specified by the funder, completing quarterly funder reports, and will serve as liaison to the funder.
· Facilitate interviewing, hiring, and coordinating, and/or directly training, new staff members.
· Responsible for designing individual and group training opportunities for staff based on needs of program and identified development needs for staff.
· Will ensure the completion and compliance of all clinical documentation within span of control.
· Ensure assigned staff complete required productivity targets and documentation on a timely basis, including both clinical and non-clinical documentation as applicable.
· Oversee the evaluation and service planning process, resource development and any other task needed to ensure adequate supports are in place post-transition, so clients will remain safe and successful in the community; Ensure all services provided are in accordance with client’s individual service plan.
