The DuPage County Health Department would like to announce an opportunity to work on our Community Support Team which provides a team-based approach towards recovery and resiliency oriented, intensive, community-based rehabilitation and outreach service for adults. The Community Support Team provides a high level of support to clients that struggle with engagement in their in Behavioral Health treatment.
This team collaborates to support these clients as one part of a comprehensive community behavioral health system that includes crisis services, adult intensive services/residential services, forensic behavioral health, substance use treatment, and psychiatric services. The Community Support Team is designed to meet the educational, vocational, residential, mental health, co-occurring disorders, financial, social and other treatment support needs of the recipient. Interventions are provided primarily in natural settings, and are delivered face to face, with individual recipients and their family/significant others as appropriate to the primary well-being and benefit of the recipient. It is provided to recipients to decrease hospitalizations and crisis episodes and increase community tenure/independent functioning; increase time working, in school or with social contacts; and personal satisfaction and autonomy. Through clinical interventions and supports based on identified, individualized needs, the recipient will reside in independent, semi-independent, or family living arrangements and be engaged in the recovery/treatment process. The purpose/intent of Community Support Team is to provide specific, measurable, and individualized mental health rehabilitation interventions to each person served through the reduction and management of symptoms and the development of stability and independence. The preferred candidate will be able to assertively and compassionately engage a population that struggles with engagement. This position involves 60% of client services provided in the community, with some office-based sessions. This position will provide an opportunity to engage in advocacy, education, and referral services to clients with serious mental illness. In this role, you will also need to have knowledge of harm reduction practices, stages of change and motivation enhancement treatment. Our new team member will have the ability to work within a collaborative team approach and work collaboratively with community providers in order to achieve client goals. This Full-Time position offers a full, DuPage County, benefit package which includes paid training, 12 paid holidays, 2 weeks of paid vacation, sick and personal paid time. This is also a pension eligible position which means there is a guaranteed retirement savings plan.
