The CENTER for Human Services, a leading provider of behavioral health care services, is seeking a Behavioral Health Case Manager – Medical (Part-Time).

Responsibilities:

• Complete screenings to determine client eligibility for services
• Complete comprehensive mental health assessments and develop treatment plans for clients receiving psychiatric services

Qualifications:

• Bachelor’s Degree in Psychology, Social Work or related field
• Credentialed as a MHP (Mental Health Professional) in the State of Illinois required
• Experience in community behavioral health preferred
• Excellent organizational, documentation, and communication skills
