Founded in 1899 as one of Chicago’s original settlement houses, Association House has a 122-year history of meeting the most pressing needs of those in the communities we serve. POSITION SUMMARY
Provides oversight, coordination, leadership, training, and evaluation of Comprehensive Class Member Transition Program (CCMTP) Care Management, RN and OT Staff. Oversees all administrative and clinical aspects of CCTMTP assessment and service planning processes, including tracking assessments and completion dates, training staff to complete assessments and service plans using Caspio Web Application, reviewing, and providing feedback on clinical documentation, and developing and maintaining working relationships with nursing facility staff. Monitors delivery of care management, RN, and OT services to Class Members. Provides direct assessment, service planning, care management services, and direct case management services to Class Members as needed. Documents all services using program protocols. Represents agency and program effectively in the community, and assists in building collaborative relationships with other providers, teams, facilities, and entities.
