Essential Duties:

· Oversee Joliet and Aurora-based mobile services, including harm reduction and medication assisted recovery units.

· Support and maintain all community partnerships established by the Senior Leadership team. Introduces Outreach workers to community organizations to build partnership relationships and coordinate this focus to generate growth to Family Guidance Programs.

· Contribute to the development of the Community Outreach Work Plan, along with members of the Senior Leadership Team. Log all Community Intervention billable hours completed by outreach team and submit to Associate Administrator on a monthly basis.

· Conduct outreach to connect individuals to substance use, harm reduction, and treatment services.

· Provide Community Intervention services in the field weekly at Mobile Van scheduled visits, by supporting outreach staff with patient engagement, and referrals to MAR services.

· Evaluate the success of our strategies, and makes recommendations to improve outcomes.

· Assign existing Community Partners to designated Outreach Team members.

· Align our weekly Outreach activities to support visits of persons needing MAT services to our Mobile Van. Schedule all Mobile Van visits in teams and send weekly confirmation correspondences via email.

· Supervise outreach teams that provide daily on the ground outreach services in the community.

· Participate in screening applicants, conducting interviews, training new employees, monitoring performance and conducting performance reviews for Community Outreach Team.

Work Schedule: Monday - Friday (7:00a - 3:30p)

Pay Rate: $50,000 - $55,000

Experience and Skills:

Education and/or Experience: Bachelor’s degree or equivalent, or a minimum of five years’ experience in evidence-based substance use disorder treatment services required. Experience in behavioral health, community health outreach, substance use harm reduction, withdrawal management programs, medication-assisted treatment, and/or mobile healthcare services required.

Certificates, Licenses or Registrations: Driver’s license and automobile insurance required.
