POSITION SUMMARY
The ACT supervisor provides dynamic leadership and supervision to a multi-disciplinary community-based team. The team provides evidence-based, long-term, intensive supports to adults with chronic mental illness and co-morbid conditions. Supervises delivery of wrap-around case management, therapy, skill building, substance use, physical health, and crisis intervention services, providing a pivotal, reliable, consistent level of support to individuals at various stages of recovery through a small, shared caseload model. The team plays a central role in the lives of the program participants, many of whom have minimal natural supports. Guides collaboration and integration of primary care, specialist care, and psychiatric care into the team delivery of services. Manages all aspects of team performance, including documentation, fidelity to the ACT model, accreditation and Medicaid standards, program outcomes, and clinical development and performance of staff. Contributes to the program’s success through on the ground direct care to participants while cultivating a highly cohesive team. Works closely with other programs and service providers in the community to increase service access and to strengthen community relationships. Develops risk mitigation and crisis prevention strategies through a creative, harm reduction approach. The position provides flexibility and the opportunity to remain rooted in direct clinical work while leading a unique, interconnected team.
