Essential Duties:

• Provide mobile outreach to at risk, unfunded individuals in targeted areas to facilitate access to mental health treatment services
• Educate community members and organizations on mental illness, the benefits of treatment, and resources available with the goal to inform and reduce stigma
• Screen at risk individuals in the community using the PHQ-2 depression screen, the PCL-5 trauma screen and the CSSRS brief suicide risk screen
• Provide mobile comprehensive mental health assessments
• Determine need for services and level of care including using the LOCUS
• Coordinate direct linkage to FGC for ongoing mental health therapy services
• Provide evidence based mental health therapy services for partial caseload in person or virtual
• Coordinate referral to psychiatric medication monitoring services and consult and coordinate care as appropriate with psychiatric providers.
• Communicate and engage with other mental team members and outreach workers and peers ensuring coordinated workflow and teamwork
• Consult and coordinate with SUD treatment staff for treatment of individuals also enrolled in FGC’s SUD treatment services

Work Schedule: Monday - Friday (8:00a - 4:30p) with flexibility to accommodate agency's needs

Work Location: 2630 S. Wabash Ave., Chicago, IL

Pay Rate: $60,000 - $75,000

Experience and Skills:

Education and/or Experience: Bachelor’s degree in associated field required. Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution required. The Mental Health Therapist is a Qualified Mental Health Professional [QMHP] who has the education, skills and experience to conduct a clinical assessment, develop a plan of treatment and provide treatment for a person with mental illness, under the supervision of a Licensed Practitioner of the Healing Arts [LPHA].

Certificates, Licenses, Registrations: State of Illinois clinical licensure required (i.e. Licensed Clinical Professional Counselor, or Licensed Clinical Social Worker). In addition to the above licensure, IAODAPCA Certified Alcohol and Other Drugs Counselor certification or proven experience with substance use treatment preferred. Clearances from CFS 689 Child Abuse and Neglect Tracking System, and CFS 708-C Background Check for Non-Licensed Contract Staff.
