Position Summary
Ensures coordination of services and continuity of care of residents. Develops and establishes intensive outreach strategies to maintain program capacity. Ensures adherence to the DHS and COA residential requirements. Monitors delivery of behavioral health services and ensures continuous quality improvement in relation to program goals and standards. Works closely with other programs and service providers in the community to increase participants’ access to needed services and to strengthen community relationships. Monitors program staff performance and the quality and documentation of their services. Identifies and seeks program development opportunities, and actively participates in bringing these opportunities to fruition.
