Job Summary
Provides a full scope of primary and preventative health care services to all adults and children to meet the health care needs of clinic patients. Direct clinical patient care activities to equal 95% and administrative activities to equal 5%. Scheduled hours are Monday, Tuesday, Friday from 8:00am – 4:00pm. Wednesday from 7:30am – 6:00pm. Thursday from 8:00am – 1:30pm.
MINIMUM QUALIFICATIONS
Current American Board of Family Medicine (ABFM) or American Osteopathic Board of Family Physicians or Board-eligible. Experience prescribing suboxone.
REQUIREMENTS
Must possess a valid Illinois Driver’s License with minimum auto liability insurance. Verification of U.S. citizenship or employment eligibility.
DESIRED
Bilingual in Spanish. Full scope practice (pediatrics-geriatrics).
