The CENTER for Human Services, a leading provider of behavioral health care services, is seeking a Continuum of Care Specialist. (Full-Time)

Responsibilities:

• Work with CiCoC administrators, ESG-funded programs, and various stakeholders to review policies, evaluate outcomes, and address gaps in services

• Support CiCoC leadership in coordinating community-wide efforts to improve outcomes for populations at risk of or currently unhoused, with a focus on reentry programming and collaboration with the Illinois Department of Corrections

• Monitor program participants to ensure compliance with CiCoC reporting standards

• In cooperation with CiCoC administration and HUD field liaisons, identify and coordinate annual and quarterly training needs, develop agendas, and implement training programs

• Provide ongoing monitoring and reporting of outcomes and systemic changes within the CiCoC

• Serve as chair of a policy review subcommittee to ensure compliance with federal and state guidelines

Qualifications:

• High School diploma or equivalent required
• Associate's degree in human services or related field preferred
• Two years experience in a field related to social services, or any equivalent combination of education, experience, and training
• Lived experience of being at risk or unhoused and/or navigating mental health, substance use systems, or other social services preferred
• Familiarity with state and federal regulations related to CiCoC, ESG, and HUD programs
• Proficiency in Microsoft Office Suite, database management, and virtual communication tools
• Must have reliable transportation, valid Illinois driver’s license, and current liability insurance

Salary Range: $19.00 - $24.00 per hour
